Anatomiz3D Healthcare Limited (“Anatomiz3D”, “us”, “we”, “our”), is committed to protecting your privacy. We own and operate the website located at www.Anatomiz3D.ca  (“Website”).

 

This privacy policy (“Privacy Policy”) describes how we collect, store, use, and distribute your information, including Personal Information, when you use the Website. For the purposes of this Privacy Policy, "Personal Information" means any personally identifying information about an individual. Please read the following carefully to understand our practices regarding Personal Information of users of the Website (“User”, “you”, “your”). Personal Information collected, used, and processed when you use some of the Anatomiz3D services offered through the Website (“Services”) may be subject to separate terms and conditions you will enter into with Anatomiz3D before accessing and using such Services.

We encourage you to review this Privacy Policy each time you use the Website to see if the Privacy Policy have been changed, as indicated by the “Last Updated” date set out above. We may, at our sole discretion, highlight the fact that we made material changes by posting a notice on our Website. Any non-material change (such as clarifications) to this Privacy Policy will become effective on the date the change is posted and any material changes will become effective thirty (30) days from their posting on the Website. Anatomiz3D may require you to provide consent to the updated Privacy Policy before further use of the Website is permitted. If you do not agree to any such change(s), you shall stop using the Website. Otherwise, your continued use of the Website constitutes your acceptance of such change(s).

 

Important Note for Patients. Healthcare professionals such as a licensed physician and/or surgeon, registered nurse, or research associate (each an “Institutional User”) may provide us patient information on behalf of their healthcare institution (each such institution a “Healthcare Institution”) in connection with the Services. If you are a patient of a Healthcare Institution, this Privacy Policy does not apply to you as your respective Healthcare Institution is responsible for the collection, use, and processing of your Personal Information. Please contact your Healthcare Institution to determine how they collect, use, and process your Personal Information.

 

1. Consent. By using the Website, you consent to the collection, use and disclosure of your Personal Information as described in this Privacy Policy. Except as set forth in this Privacy Policy, your Personal Information will not be collected, used, or disclosed for any other purpose without your consent. We will not actively collect Personal Information for the purpose of sale or marketing of such Personal Information in a way that specifically identifies an individual. You may withdraw your consent to our processing of your Personal Information at any time. However, withdrawing consent may result in your inability to continue using all or parts of the Website.

 

2. Information You Provide Us. You may disclose information to us as follows:

 

a) By Using the Website. You may provide us with certain Personal Information when you use the Website. For example, you may submit feedback, comments, questions, or requests to be contacted by Anatomiz3D via the Website (“Feedback”), or you may submit education details to determine if you are eligible for a discount for educational courses on medical 3D printing.  The Personal Information we collect may include your first and last name, applicable Healthcare Institution (as applicable), relationship to applicable Healthcare Institution (as applicable), email address, phone number, any Personal Information you decide to include in your Feedback to us, your profession, your degree, your state, your country, and any other Personal Information you may provide to us while using the Website.

b) Supplying us with goods or services. If you are a supplier of Anatomiz3D, the Personal Information we collect includes a contact name, email address, business address, and phone number.

c) Applying to work with us. If you apply for a job with us, the Personal Information we collect includes your first and last name, email address, phone number, address, your references, and any Personal Information about them that you provide to us, your resume, and any Personal Information in your transcripts, certifications or other hiring materials you submit to us.

d) Using Anatomiz3D’s professional services. If you engage Anatomiz3D to perform development work or other one-time services, the Personal Information collected includes your name; email address; phone number; address; and billing and payment information.

 

If you submit your resume or other hiring materials to us, we ask you to only include information relevant to the position you’re applying for, and your employment history and education (including, without limitation, certifications and/or degrees obtained, previous work experience, relevant awards, etc). If you submit Feedback or any other materials to us in connection with your use of the Website, we ask that you do not disclose sensitive Personal Information (including, without limitation, gender, height, weight, medical information, religion, and philosophical or political beliefs) in your submitted materials.

 

3. Information We Collect When You Use the Website. Anatomiz3D may collect certain information when you use the Website, such as internet protocol addresses. This information is logged to help diagnose technical problems, and to administer and improve the quality of the Website.

 

We may use cookies to track content usage and traffic on the Website. A cookie is a feature of your web browser that consists of a text file that is placed on your hard disk by a web server.
